**Job Summary**

AECOM is actively seeking a creative, highly talented Case Manager for immediate employment. This is a part-time variable position, meaning work days and times will be determined on an as-needed basis.

The responsibilities of this position include, but are not limited to, those listed below:

+ Case Managers accomplishes clients' care by assessing treatment needs; develops, monitors, and evaluates treatment plans and progress; facilitates interdisciplinary approaches; monitors staff performance.

+ Review records and applications; confirming program eligibility.

+ Determines clients' requirements by completing intake interviews;

+ Coordinating services being provided; arranging resources.

+ Monitors cases; obtaining additional resources; intervening in crises; providing personal support.

+ Maintains clients' records by reviewing case notes; logging events and progress.

+ Communicates clients' progress by conducting weekly interdisciplinary meetings and evaluations.

+ Prepares clients' closeout by coordinating program requirements.

+ Prepares reports by collecting, analyzing, and summarizing information and trends; compiling statistics.

+ Enhances department and organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.

**Minimum Requirements**

+ High school diploma or GED

+ 2+ years of work experience in client facing jobs requiring communications to both customers and outside entities (vendors, etc).

+ Must participate in and successfully complete a post-offer, pre-employment fitness for duty questionnaire.

+ Must possess a valid driver's license and clean driving record and required auto insurance

+ Excellent Verbal and Written Communication proficiencies

+ Strong Documentation Skills…capturing details accurately and coherently

+ Ability to analyze multiple data elements and make decisions;

+ Strong judgement/decision-making skills…ability to use information available and make decisions.

+ Research Skills…resourceful, able to seek out information independently

+ People Skills…ability to adapt style to variety of people; capable of interacting effectively with general public

+ Resolving Conflict…ability to resolve conflict quickly and effectively

+ People Management

+ Integrity

+ Physically able, with or without an accommodation, to work in an indoor environment. Position requires frequent keyboard/mouse operations and sitting for long periods of time; rarely lifting less than twenty-five pounds, standing/walking, climbing stairs, performing repetitive fine hand movements.

**Preferred Qualifications**

+ Bilingual in Spanish and English
